It only happened to me once, and it was the first attempt. I had my Super Color Bomb active, so there were big explosions at the beginning of the level, and probably massive cascades too. My experience seems to be similar to wonderbison's.

'Players you have teamed up with' section seems to appear after leaderboard events and contains players from them. In my cat game during All Stars, it had some of the players from the previous two rounds.
