I'm new. Where do I send the screenshot of over 5,000 games?
Also, is there a Champion designation on our profiles after we win three weeks worth of games? After finally getting it, it was really anticlimactic.
As for Championship Streak, you receive badges that keep track of your wins in weekly contest.
Once you achieve 3 consecutive wins, you unlock the Championship Streak prize and badges reset so you can earn 3 more and unlock the prize again.
Most badges can be collected for the community account, like milestone level badges
The only "badges" that can be collected and kept in game profiles for bragging rights are trophies from Soda Cup. Each Soda Cup round has it own designed trophy and one trophy can be earned once players reach a certain threshold of points in a Soda Cup round.

To obtain the badge for your community account (not your game account), please take a screenshot of your Soda map showing your current progress, open the link below to find the discussion thread for level 5000 badge and post your picture in the comments.
With the same picture you can claim badges for level 1000, 2000, 3000 and 4000 in their own discussion threads.
As you achieve new milestone levels, you can come back to the community and claim more milestone level badges.
