The form will appear as a pop-up! So, please keep an eye on all the pop-ups you see in the game.
Play a level and finish it - then come back to the Map screen. A pop-up should appear asking you to fill the form.
If nothing happens, please close the App - restart your device - reopen the App - replay / play a level and the Form should appear.
