I have been using facebook sign-in to play Candy Crush Saga and I have been seeing my facebook photo when I played Candy Crush Saga. I didn't make any changes. But yesterday, my avatar somehow got changed to this helmet-waring frog. I signed out and signed back in with facebook but the avatar stayed the same.
@billcheng, sounds like your game has become disconnected from the server. please try checking for a game update and logging out and restarting your device and then log back in and see if your avatar returns , also try playing a level to make sure it connects to server correctly

Thanks for the suggestions! I stopped the app, cleared the cache, restart, and I got the avatar back!

I'm glad that worked for you. Those standard troubleshooting steps are always the first thing to try. Have fun.
To be frank, I would consider that a software bug (or some kind of configuration bug)! I think the helmet-wearing frog is a stock avatar. May be the stock avatar should always be set to have a timestamp of 0. This way, any newer avatar will replace what's in the cache.
