That is correct, you are given ONE extra life and a challenge to win on that attempt, @mteach. You get five lives IF you win on that attempt. If you lose, you get no lives, only timed boosters.

The fishing boat doesn't give you two lives immediately. Instead, it offers you a chance to win two lives. You are given an extra attempt, and you must win the level on that attempt. If you win, you'll see this screen and receive two lives.
If you fail, you will get no lives, but you will keep the timed boosters.
