Incomplete FB login page

I'm trying to help my friend with a FB login problem. She has been logged out of Soda and when she tries to log back in again, she only sees the FB "Confirm" banner. No sign of the Cancel/OK buttons.

I've tried the delete app/ FB app suggestions to no avail

iPhone 6S+. Her iPad mini has no problems

Any suggestions?

Is this in the Facebook app or on the iPhone. On my home screen I have a 'connect' button below the play button but that's all I have. I don't see any ' confirm or cancel buttons so I'm not quite sure what you are referencing. Post up a screenshot if you can to make it easier to see.
