I have purchased a special offer (11th July) but have not received the goods. This is the 2nd Time this has happened in last few months even though I have had confirmation from iTunes that the purchase was successful

While filling out the form given in above link, make sure to specify the game you have bought the package. And then, enter the Topic as "Problems with Purchase" and Sub-Topic with "Purchased Items not received". Then explain about the issue in provided comment box.
You will receive an automated email with a ticket number as an acknowledgement. If you have not heard from them within next 3 days, feel free to send a reply through that automated response.
