My candy crush game has disappeared from Facebook, anyone else have this issue?

I also lost all my King games on Facebook. It says User opted out of the interface. I would like to reinstate my level here but cannot find out how. Google says look for the SETTINGS icon, but there isn't one. HELP!!

I also play via Mac laptop. Never had any issues until yesterday.
