Sometimes the badge appears after passing a few levels after the Milestone level. Try to pass a few levels and then repeat level 5000.

I am currently at 5046 and have went back beaten level 5000 many times and still have not received my banner

Same for me except I’m past 10,000. No badge. 100+ levels past it.
