It has been few hours since you posted this question! Did you receive the form yet?

If not, have you tried the trouble shooting steps? If you have been playing continuously, take few minutes break — close the App - restart your device after shutting it down for about 5-10 minutes — reopen the App — play an old level and lose it — go back to the map screen and the form should pop-up 👍

You won’t miss it as it is big pop-up that will come up with Yeti.
