Have you been playing the game continuously?
I would suggest you to close your game for few minutes - shut down your device for about 5-10 minutes and restart it. Now, open the game and you should see the Form delivered by Yeti.
If you still didn't see it, replay an old level and lose it. Then, go back to the map screen and check if the form is there yet!
