On your old phone, did you save the game either with Facebook or King (email) account? If so, all you need to do is — download the game on your new phone and log back in with the same account you have used on your old phone.
Both your level progress and the Gold Bars will be transferred as these are saved to the King servers. But, your boosters and lives will not transfer because there are stored to the local memory of the device we are using, which is not transferrable!
What I would suggest you is — not delete the game on your old phone, but play the game and use up those boosters to win hard levels, while collecting boosters on your new phone.
