Have not received my form yet either. Have not been in first the entire round but have been for most of it. Has anyone that posted this since got their form?

Hi everyone! Just want to reassure that it's no worries if you haven't seen the form in game yet or if you missed it. No need to contact player support, just keep playing and the pop up will show up again. We'll send it out a few times so everyone has the chance to fill out their information 😊

Hello :)

I'm in Final Knockouts and still haven't received the pop-up eligibility form.

I'm excited about the possibility to go to finals and am currently #1 in my group.

Is there any other way to receive the form? Perhaps accessing it on Facebook or something?

It seems that others are also having difficulty.

I have received the form but i am from ethiopia and the form did not have my country on the list. am i not eligible to participate in all star? please let me know.
