In order to get the Milestone banner for your avatar, you need to open the app and then click on the Pink and white wheel in the upper right hand corner. That will open up your game settings. Then click of the green check mark that says features and then one of the options says Milestone Badge. Make sure you have activated that feature (click and get a green checkmark), then go play a level and the new banner should appear.

Hi - Welcome to our friendly community. All you have to do to get/keep the 5000 level Milestone badge is to take a screenshot of your map showing your avatar and post it here. You are eligible to get all the milestone badges thru 12,000 and you can use the same map screenshot (showing you are at 12,435) by posting it in each of the threads here: Level 1000 Level 2000 Level 3000 Level 4000 Level 5000 Level 6000 Level 7000 Level 8000 Level 9000 Level 10 000 Level 11 000 Level 12 000
Just click on each level and post your game map screenshot and you will get the badges you have earned.
