In order to get the Milestone banner for your avatar, you need to open the app and then click on the Pink and white wheel in the upper right hand corner. That will open up your game settings. Then click of the green check mark that says features and then one of the options says Milestone Badge. Make sure you have activated that feature (click and get a green checkmark), then go play a level and the new banner should appear.
