How do you get the application form for the All-Stars final competition??

It states states in the terms and conditions that you need to send an application form before the end of the third final knockout round. Where do I get this application form?

All Stars can disappear from anyone's game if you inadvertently changed the time/date on your device to before or after the round starts of finishes. That may be the most likely explanation for anyone losing the event. The event also disappears if you travel outside of a country that is part of All Stars or if you use a VPN. King may also disqualify players that their systems find "suspicious" plus there may be glitches. In my experience, if the event does not show up once the next round has started there is nothing a player can do to bring it back. It happened a lot last year and it will happen again this year.

Hi  I placed 2nd in the 2nd knockouts .Today the final started and I don't have it anymore  Even my purple candy's are not stars anymore I would appreciate any help or answers
